This receipt is valid for a dog that has been in a rabies high-risk country in the 6 months before entering the United States. This receipt may be used only for the dog described below for a single entry to the United States on the date of arrival listed above.
This receipt does not guarantee a dog's entry into the United States. You must show this receipt (printed or on your phone) to airline staff and/or U.S. port of entry officials. Additional documentation must also be shown to airline staff and/or U.S. port of entry officials to verify a dog's eligibility for entry into the United States. CDC reserves the right to take other actions to protect the public's health.
FOR DOGS THAT HAVE BEEN IN A HIGH-RISK COUNTRY FOR RABIES IN THE PAST SIX MONTHS, ONE OF THE FOLLOWING IS NEEDED IN ADDITION TO THIS RECEIPT:
- A valid Certification of U.S.-Issued Rabies Vaccination form; OR
- A USDA-endorsed export health certificate (valid until the rabies vaccine listed on the certificate expires, as long as the certificate was issued no later than July 31, 2025); OR
- A valid (not expired) CDC Dog Import Permit valid for entry before August 31, 2024. You should arrive at the Port of Entry that matches what is on this receipt. Your dog does not need a reservation at an animal care facility if you have a permit; OR
- A reservation for examination and rabies vaccination at a CDC-registered animal care facility upon :
arrival.
o Foreign-vaccinated dogs that have been in a high-risk country in the past six months and
do not have a CDC Dog Import Permit (see #3) must have a reservation at a CDC-registered animal care facility for examination and revaccination immediately upon
arrival at the importer's expense.
o Foreign-vaccinated dogs that have been in a high-risk country in the past six months and do not have a CDC Dog Import Permit (see #3) must also have a valid rabies serologic titer from a CDC-approved laboratory or undergo a 28-day quarantine at a CDC-registered animal care facility for examination and revaccination immediately upon arrival at the importer's expense.
o Foreign-vaccinated dogs that have been in a high-risk country in the past six months and do not have a CDC Dog Import Permit (see #3) must also have a valid rabies serologic titer from a CDC-approved laboratory or undergo a 28-day quarantine at a CDC-registered animal care facility at the importer's expense.
For dogs that have been only in dog rabies-free or low-risk countries in the past 6 months, no additional documentation is required for entry.

The reservation is made once you send all the documents and the corresponding payments, we review and if they are OK then we proceed to the reservation, please send all the documents that we require at least 10 days before your arrival.

These are the requirements for arrival beginning on August 1, 2024. Please read the entire email carefully, as most of your questions will be answered. Please carefully review the information below, which includes the method of payment accepted and attachments to be completed.
Steps to start your Pet's Reservation:

In order to guarantee a reserved space for your arrival date, please send the minimum required $350 non-refundable deposit, which will be deducted from your total balance. Your reservation balance MUST be paid in order to receive the official confirmation reservation email.
Once we receive your deposit we will begin to review the documents. Your reservation is not complete until you receive the final reservation confirmation email.

Health certificate - Expires 10 days from the issue date, please ensure it is dated within 10 days of arrival (Must either be in English, include English translation on certificate or you must provide a translated copy of certificate by a certified translator)
Passport of person flying with the pet or Consignee
Passport of Owner (if different from the person traveling with the pet)
4 Photos: Front facing picture of your pets face, front of teeth, side of teeth and a side picture showing the full body of your pet
Proof of Flea & Tick preventative (usually listed on the health certificate or vaccine records)
Serology Exam (If applicable)

Obtain a licensed broker to clear your pet through Customs after we receive the CDC release for your pet. Once obtained, please send us the name & phone # of your broker. The cost of the broker is not included in our fees. In Miami, ALL pets require a broker to clear the customs process, at your expense, no matter the method of travel. You may choose any broker you prefer as long as they can clear customs in Miami and pay all the required airline fees.
Program Procedures:
- Pet must arrive through Miami International Airport; we must pick up your pet from the passenger or cargo area for transportation to our bonded quarantine facility. You may NOT leave the airport with your pet. Your pet can only leave the airport in our custody. If you leave the airport with your pet, you are in direct violation of the CDC program requirements and will incur additional fees
- The following business day, the USDA Vet will examine the pet and administer the required US rabies vaccine.
- CDC release is requested. Once received, it is sent to your broker for Customs clearance. The Customs clearance is what usually takes the longest.
- We will contact you to pick up your pet, after we receive Customs clearance.
Quarantine Programs Available:

If you already have an approved Serology/Rabies Titer Exam drawn at least 28 days prior to arrival date - $1,300 (Quarantine Program Completion time is 2-3 Business Days after arrival) The process includes veterinary exam and re-vaccination, CDC release and Customs Release. Due to the different departments and agencies involved and our previous experience, in Miami, it usually takes 2-3 days.

If you do not have an approved Serology/Rabies Titer Exam prior to arrival date - $3,220 (Quarantine Program Completion time is 28-30 Business Days after arrival)

Our transportation department can also assist you in delivering your pet to your final destination, by ground or air, whether it's local, domestic or international.
Our in-house USDA certified veterinarian can assist with any additional travel documents to the final destination, including the USDA Vaccine Certification, if needed for future travel.
You acknowledge that aside from your $350 non-refundable deposit, any fees for services used also become non-refundable and any refunds will be calculated based on unused services in the event of a cancellation.
In the event of airline delays or flight changes, we will adjust the date of arrival at no additional cost.
