First you will need to have the national board forward your passing score report to this office. Contact the American Dental Association to have your score report sent to this office. If you have not taken the National Board Dental Hygiene Exam or the National Board Dental Exam (both parts) you will need to go ahead and take one or the other. The number for American Dental Association is 800-232-1694.
You will then need to submit the "credential review application", found on our website and attached, along with your dental diploma & transcript and contact the ECE (Educational Credential Evaluators) at www.ece.org and have them do a "subject analysis evaluation". They can do also do a "course by course evaluation" if it shows all courses for 4 years. The diploma and transcript you send do not have to be translated. The ECE phone number is: 414-289-3400.
Once your credentials have been approved, you will then be approved to take the ADEX and the Florida Laws & Rules Exam through The CDCA (The Commission on Dental Competency Assessments) formerly the NERB (North East Regional Board) and your name will be sent as being approved for the exams. For the laws & rules exam, you will be tested on Chapters 456 and 466, Florida Statutes and Rule Chapter 64B5, Florida Administrative Code.
After you have taken and passed your exams, you will need to submit the application for licensure and the fee of $135.00 and a copy of your current CPR basic life support training card.
Your CPR certification must be the basic life support for healthcare providers and cannot be taken online.
